Nurses at Riverside Health Care Center discovered Mae Burke, 86, holding a bottle of toilet bowl cleaner to her mouth Jan. 20, Administrator Rob Giannini said Wednesday.
Burke, who suffered from dementia, was immediately taken to Danville Regional Medical Center. She died there five days later.
A state inspection May 27 confirmed that cleaning chemicals were left unattended on a housekeeping cart, said Norman King, director of the special services unit of the Office of Health Facilities Regulation in Richmond.
