Carman Allen Middlebrook, 69, a devoted and loving father, died April 25, 1995, in Chesapeake.
Mr. Middlebrook was born in Toronto, Canada. He was the widower of Avis Olson Middlebrook. He was a member of Berkley Masonic Lodge 536 F & AM, Berkley, Mich.
Survivors include a son and daughter-in-law, John D. and Tammy T. Middlebrook of Norfolk; a granddaughter, Hailey H. Middlebrook of Norfolk; and numerous relatives in Canada.
The funeral will be conducted at 7:30 p.m. Tuesday in Hollomon-Brown Funeral Home, Tidewater Drive Chapel, by the Rev. Dr. John Monroe. Burial will be at 10 a.m. Saturday, May 6, 1995 in Michigan Memorial Park. In lieu of flowers, memorials may be made to Alzheimers and Related Disorders, 20 Koger Center, Norfolk, Va. 23502.
